The RN works under the direction of DON, provides skilled professional nursing care to the patient as prescribed by the physician and in compliance with State and Federal regulations, development, maintaining of the patient plan of care/Coordination, and the policies of Divine Home Care of Ohio. [EDUCATION]: Graduate of an accredited school of professional (registered) nursing. [LICENSURE]: Current Ohio license. [EXPERIENCE]: One year recent experience in an acute care setting, home health experience preferred, BUT we’re willing to train. [CERTIFICATIONS]: Current CPR certification. [SPECIAL SKILLS]: Competent in IV therapy, venipuncture, assessment, patient teaching skills. [OTHER]: Possess current driver’s license and auto liability insurance. Has excellent observation skills, knowledge of the nursing process and communication skills. This position puts you in a supervisory role over home health aides and LPNs.
We are looking for a Licensed Practical Nurse (LPN) to assume responsibility and accountability in the delivery of patient care, under the supervision of a Registered Nurse (RN). The Licensed Practical Nurse (LPN) will consistently perform according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.
Responsibilities: Performs focused interview to identify specific patient needs; Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition; Collects, analyzes, and interprets data and information from health care members and documents patient conditions and progress; Coordinate patient care / clinical changes with physician, DON, RNs, QAs as needed; Other Licensed Practical Nurse (LPN) duties as assigned.
